MCYSA GAME CARDS

Note that there are some changes to the cards this year.  Still 4 sections but you only return Section C to the coaches:

Our game cards are 8-1/2 x 11 full sized sheets on heavy weight paper.

Each card is scored into 4 sections.

You will receive a Game Card from each coach before the beginning of each game.

Once the game is over YOU will leave with the Sections A and B and D . . .

RETURN Sections C (only) to the COACHES of opposing teams.

 

 SECTION A
TOP HALF  (SECTIONS A & B)  KEPT BY REFEREE

TEAM ROSTER

CONTAINS:  Team Name, Coaches Names, and Player Roster

Coach:  The coach should have this completely filled out prior to the game.
Referee:
  Make any corrections to roster - cross out coaches/players not in attendance, mark those serving suspensions with a "S".  There is also 2 new columns "Y" and "R" where you can mark cautions and send offs using the appropriate 7 & 7 code in the Y or R Column.

CONTAINS:  Formatted to send as postcard to Joe Marrapodi

Referee:  Card A to be MAILED to Joe Marrapodi

 SECTION B

GAME INFO

CONTAINS:  Game Number, Home/Away teams, Date Played, Start & End Time, Score Area, Sage Check Off, Coaches Signatures

Referee:  Fill out completely at the field. 
Coach:  Both COACHES SIGN at the end of the game to show agreement with score

REFEREE DISCIPLINE REPORT

CONTAINS:  Contains areas to note team/coach/fan conduct and any infractions resulting in Red/Yellow card

Referee:  To be completed by referee at field.  You will need both sides of this card to SUBMIT the ONLINE GAME REPORT once you get home.

BEFORE GAME

TEAM ROSTER

CONTAINS:  Team Name, Coaches Names, and Player Roster

, mark those serving suspensions with a "S".  This portion returned to opposing coach BEFORE game so they have a copy of their opponents roster and the Evaluation Report. (Be sure to fill in ref info on reverse before returning to coaches.)

COACHES REFEREE EVALUATION

CONTAINS:  Info for the Coaches evaluation of the game referees

Referee:  Needs to PRINT Names and ID #'s, and Center &  AR's at game and RETURN to COACH BEFORE the game.
Coach:  Submit Score report and Referee Evaluation ONLINE after the game.

REFEREES KEEP THIS SECTION

GAME REPORT BY REFEREE

CONTAINS:  Information about the location of the game, field conditions, and comments/major problems

Referee:  To be completed by referee at field.  You will need both sides of this card to SUBMIT the ONLINE GAME REPORT once you get home.
