| Card A - this is the one with the TEAM ROSTER. Card A must be MAILED to Joe Marrapodi immediately after each game. The card is set up as a postcard with Joe's address on the reverse side of the roster. | Card B has the MCYSA SCORE CARD on one side and REFEREE DISCIPLINE REPORT on the other. Both sides of Card B is submitted online using one of the forms linked above. | Card D which has the GAME REPORT BY REFEREE on one side and game reminders on the other. Card D is submitted online using one of the forms linked above. |
