MCYSA FALL 2010:  TEAM REGISTRATION APPLICATION (ver 10.2b)

It is the responsibility of the League Contacts, acting as representatives of their clubs, to assure that all teams from their club are registered and that submitted information is complete and accurate.  League Contacts may designate others to complete and submit the form below.  League Contacts are sent copies of all submissions for their club.

  • Fill in form Completely and Accurately.

  • When finished, click the SUBMIT button at the bottom of the form.

  • Submissions with incomplete data or invalid team names will not be accepted and team will not be registered.

  • U08 through U14 Teams not registered as of July 15th will be put on a Wait List and may not be eligible to participate in the Fall season.

  • Registration deadline for Division 1 & 2, U15 through U19, is September 1st.

Items from NJYS you should be aware of during registration:

1) NEW U08 and U09 players may only play on small sided teams

2) NEW U10 players may only play on a U10 or U11 team

3)  In order for a team to register and play as U11 please note that all U11 teams MUST have a MAJORITY of U11 players at all times

4) Players U07 and younger can NOT register for travel soccer. 

5) DIVISION 1 (U17, U18 & U19) team should be aware of the new 22 player roster information (effective August 2005):

TEAM CONTACTS ~

You will be asked to list the Head Coach, Primary Contact, and Alternate Contact for the team.  The Primary and Alternate Contacts must be two different people, must have separate phone numbers and email addresses OF THEIR OWN, and may not be members of the same household.   Otherwise it defeats the purpose of having a back up contact!  The Primary and Alternate contacts are published on the website along with their email, phone, and fax (if available).  You can designate the Coach as either the Primary or Alternate contact, however some Coaches or Coach/Trainers  prefer to appoint the Team Manager, Asst. Coach, Team Parent, etc. as the team contacts.  Please keep in mind it is the Primary Contact who is the main point of contact for the team.

If any of the contacts are the same as last season you will only need to fill in the first/last name and email address (we will pull the rest of the information from last season's records).  If any contact is different from last season, has updated information, or the team did not play last season in MCYSA you will need to complete ALL the information for each contact.
