All notification of Make-up Game Dates, Time Changes & Field Changes MUST come from the League Contacts or Field Schedulers ONLY. All schedule changes must be submitted using this form. No other form of notification or request can be honored or acknowledged.

All Change Requests must be submitted with a minimum of 7 DAYS ADVANCE NOTICE of the affected game date.
Any change REQUESTED by the REFEREE ASSIGNOR need to be reported ASAP!!
Any NJYS STATE CUP / NATIONAL COMPETITION conflict need to be reported As Soon As You Know!!
