PARTICIPATION AGREEMENT |
2008 Information
The tryouts are scheduled for Saturday,
April 19 and Saturday, May 3.. All tryouts
will be conducted at a neutral site.
The tryout format is meant to simulate game
situations using small sided and perhaps full sided games.
Try-outs will
be conducted by an independent panel of qualified evaluators.
All age groups will have two tryouts. Team
administrators will be selected after teams are selected. Tryout location
does not indicate the location of training or games. One or more teams may
be formed in each age group depending upon response and tryout results.
Players may tryout for only one age bracket.
Players are strongly urged to tryout with their appropriate age bracket.
All players must present a copy of their player pass when registering at the tryout site.
Although we strongly suggest that players
attend both tryout dates, attendance at one is required.
Pre-Registration
by EMAIL is now open until 9PM May 2nd.
To
register prior to our next try-out on May 3rd do one of the following:
A:
1. Download the Word
Application Form (click it; its a MS Word document) and fill-in the
requested information.
2. Save and Rename the document with your child's Last Name and First
Initial.
3. Email the completed application form to embonder@optonline.net.
B.
1. Download the PDF
Application Form and complete the form.
2. Scan the completed form and save as a PDF
3. Email the completed PDF
application form to embonder@optonline.net
Walk-on
registrants are accepted.
To register
on site the day of the try-out do the following:
a.
Download the Registration
Form (click) fill it out and bring it to the registration.
b. Print off and bring a completed
Participation Agreement
c.
Important: Copy of primary player pass (secondary pass if applicable)
d.
Check (payable to MCYSA) for $25.
The Select Team Tryout fee for those who
pre-register online is $25. This includes a numbered Select
Tee-Shirt for each participant (to keep) that will be worn at both tryouts
to identify the player. The registration fee will be collected at the first
tryout attended and must be paid by Check made payable to MCYSA.
No cash please!! All players must present a copy of player pass when registering at the
tryout site.
In the event that a sufficient number of
players do not pre-register for a specific age bracket, a team for that age
group will not be organized. Notification of try-out results will occur by posting player numbers on the MCYSA
website.

In the event of inclement weather, check the website 2 hours prior to the
start of the tryout.
Players MUST arrive at least 30 minutes PRIOR to your tryout for check-in.
Players arriving late may not be able to participate in the tryout.
