After submitting a Player Application, please print this form, complete, and mail with your Entry Fee.
Applications can not be considered until Legal Release and Entry Fee are received.
Print this release form and mail it with your $22 check made payable to: "M.C.Y.S.A."

Deadline for applications is June 4, 2008. No applicant will be accepted with incomplete profile information. No applicant will be accepted if we cannot verify your player experience with your club and high school coach.

Release: I certify that ____________________________________ is a Junior in High School and is expected to graduate in the spring of 2009. Also, recognizing the possibility of injury with soccer and consideration for the USYS/SS, the M.C.Y.S.A. and its affiliates accepting this application for the College Soccer Showcase, I hereby release, discharge and or otherwise indemnify USYS/SS, M.C.Y.S.A., its affiliated organizations and sponsors, their employees and associated personnel, the coordinators of the College Showcase, against any claim by or on behalf of the applicants participation in the College Showcase. I agree that I will be present during the College Showcase to authorize medical treatment if such should be necessary. The above named applicant has received a physical exam and has been found physically capable of playing competitive soccer and participating in the College Soccer Showcase.
